Waiver Release and Consent to Tattoo

I acknowledge by signing this form that I have been given a full opportunity to ask any and all questions which I might have about obtaining a tattoo by All Wolves No Sheep

I acknowledge that it is no reasonable possibility to determine whether I might have an allergic reaction to the tattoo or tattoo process and I accept the risk that such reactions are possible and I do not hold All Wolves No Sheep or its employees responsible for any allergic reaction if one does occur. If I have any known allergies I have informed All Wolves No Sheep.

I acknowledge that infection is always a possibility. In the event that I do not take proper care of my tattoo in no way do I hold All Wolves No Sheep or its employees liable for any problems or expenses that may occur if an infection does develop.

I agree to take care of my tattoo as All Wolves No Sheep has instructed with the aftercare tattoo procedures form provided.

I acknowledge that I have truthfully presented proper identification to All Wolves No Sheep and that I am of age (minimum age of 18 for tattoos) to receive a tattoo.

I acknowledge that I have truthfully presented to All Wolves No Sheep that I am not pregnant and DO NOT hold All Wolves No Sheep or its employees liable for any problems or expenses that may occur if there is an incident involving my pregnancy.

As of six months prior to today if I have had hemophilia, hepatitis (all types), AIDS (actual AIDS, not just HIV+), or have anemia, diabetes, hemophilia or epilepsy, I have discussed this with All Wolves No Sheep in terms of how obtaining this tattoo may affect my condition(s).

I acknowledge that obtaining this tattoo is my choice alone and I consent to the procedure.

I also acknowledge that I am not under the influence of drugs and/ or alcohol.

I agree for myself, my legal representatives, and/or my power of attorney to release and forever hold not accountable All Wolves No Sheep and its employees from any and all claims, damages or legal actions arising from or connected in any way to the tattoo and the conduct of the tattoo artist doing the tattoo in any and all places All Wolves No Sheep conducts business.

I agree that no refund can or will be made for tattoo services that I have received.

I agree to wear a mask or facial covering during the entirety of my tattoo in order to minimize exposure for myself and my artist, unless my artist states that an exception applies. I also agree to adhere to socially distancing and health safety practices within the shop. This includes limiting the guests, personal items, and food that I bring into the shop.

I acknowledge that I have not recently experienced any symptoms associated with Covid-19 including coughing, chills, fever, shortness of breath or difficulty breathing, new loss of taste or smell, or fever.

I acknowledge that I have not recently traveled to any states or countries that are currently under the New York state travel advisory due to rising numbers of coronavirus testing.

I acknowledge that I have not been diagnosed with, have symptoms of, or been exposed to Covid-19. I also have not been in contact with anyone who has been diagnosed with or had symptoms of Covid-19.

I certify that I am over the age of 18 and that all the information I have presented in this form is correct and true.

Deposits for tattoos are non-refundable and/or void if you miss your appointment date/time. A 48 hr notice is required to reschedule your appointment date/time. Deposits are also non-refundable if the initial design(s) have been created.
